Consent & Acknowledgement for Covid-19 Testing

You understand that by booking a COVID-19 test, you consent to:

• Haztech administering, or in the case of virtual testing, monitoring and validating the COVID-19 test (the “COVID-19 Test”);
• Haztech collecting personal information from you as required for testing administration; and
• Haztech making disclosures as set out below and any other disclosures that may be required by applicable law or regulation from time to time.

You understand that:

• Haztech is required to disclose all COVID-19 test results, including your COVID-19 Test result, and your personal information required for testing administration, such as your name and contact information, to the applicable health authority. Such health authority may contact you directly if your COVID-19 Test is positive and may conduct research using your COVID-19 Test result and personal information.
• The COVID-19 Test is not 100% accurate, especially if self-administered, and may produce incorrect results. False positives are rare. The COVID-19 Test is not intended to replace the services and/or medical advice of a licensed health professional and Haztech makes no representations or warranties as to the accuracy of the COVID-19 Test.
• False negatives are also rare when the sample contains sufficient viral particles for detection; however, because several factors may contribute to an infected person having an undetectable viral load at the time of testing, the COVID-19 Test does not rule out the possibility that you have been exposed to or are infected with COVID-19.
• You are responsible for monitoring your own symptoms and notwithstanding the results of the COVID-19 Test, must comply with the applicable directives of public health and the instructions of your licensed health professional if you experience symptoms of COVID-19.
• This Consent and Acknowledgement will be kept and retained in the records of Haztech.

YOU HEREBY RELEASE, WAIVE AND DISCHARGE HAZTECH, AND EACH OF ITS AFFILIATES, DIRECTORS, OFFICERS, EMPLOYEES, AND AGENTS OF AND FROM ALL CLAIMS, DEMANDS, DAMAGES, COSTS, EXPENSES, ACTIONS, AND CAUSES OF ACTION, INCLUDING INJURY, ARISING OR TO ARISE BY REASON OF THE COVID-19 TEST OR RELATED DISCLOSURES.

You understand that you are entitled to receive a copy of this Consent and Acknowledgment and can request your COVID-19 Test results directly from Haztech.
