Yes, you need a lab requisition to book our mobile lab service. We cannot collect a specimen unless you provide a requisition completed by your healthcare provider.
We can collect samples from anyone ages 13+. Discounts available for seniors age 65+ and palliative care patients.
To book 2 people at the same location, book your appointment and add one person after selecting your appointment. An additional person is only $30.
